Service Work Authorization

This Service Work Authorization ("Authorization") is between you (the "Customer") and MW+ Garage Door Solutions, operated by Taha Barakat ("MW+", "we", "us"). By signing below, you authorize us to perform the diagnostic and repair work described in your booking, on the terms set out here.
1. Authorization to perform work
You authorize MW+ technicians to enter the service address listed on your booking and to perform diagnostic inspection, repair, replacement, and adjustment work on the garage door, opener, springs, cables, rollers, tracks, and related components, as reasonably required to resolve the issue described in your booking.
2. Estimates are not final prices
Any online or phone estimate is based only on the information you provided and the photos we have seen. The final invoice depends on what we find on site: the specific parts required, additional damage discovered, and the time the work actually takes. We will discuss any material change in scope or price with you before continuing.
3. Payment
Payment is due upon completion of work unless we have agreed something different in writing. Accepted payment methods are listed on your invoice. Balances unpaid 30 days after completion may be subject to collection and reasonable collection costs.
4. Workmanship and parts warranty
Workmanship on services completed by MW+ is covered for one year from the date of service. Parts carry the manufacturer's warranty, passed through to you. Warranty coverage does not apply to damage caused by misuse, accidents, weather events, power surges, vehicle impact, attempted self-repair, or work performed by others after our visit.
5. Pre-existing condition and liability
You acknowledge that garage door systems are mechanical and that pre-existing wear, damage, or improper prior installation may exist. MW+ is not responsible for damage that results from such pre-existing conditions, including damage discovered during the course of normal repair work. We will inform you if we observe pre-existing issues that affect the work.
To the fullest extent permitted by applicable law, our total liability arising out of the work performed under this Authorization is limited to the amount you paid us for that work. We are not liable for indirect, incidental, special, or consequential damages. Nothing in this Authorization limits any rights you have under applicable consumer protection law.
6. Cancellation and rescheduling
You may cancel or reschedule a booking at no charge up to 2 hours before the appointment window. Cancellations inside that window, or no-shows when a technician arrives, may be subject to a trip-charge to cover dispatch costs.
7. Photos and records
We may take before-and-after photos of the work area for our service records and for warranty purposes. We will not publish photos that identify your property without your permission.
8. Customer rights and ownership
You retain ownership of all personal information you provide to us. You give us permission to use that information only to perform the work, communicate with you about the booking, and meet our legal and tax obligations. The MW+ website, brand, software, and content are owned and controlled by Taha Barakat — this Authorization does not transfer any rights to the website or brand to you.
9. Governing law
This Authorization is governed by the laws of the Province of Ontario, Canada, and the federal laws of Canada applicable in Ontario. Any dispute will be brought in the courts of that province.
10. Electronic signature
You agree that typing your full name in the signature field below is a legally binding electronic signature, with the same force and effect as a handwritten signature, under Canada's PIPEDA / Electronic Commerce Act and the United States E-SIGN / UETA frameworks. We record the date you signed, your IP address, and your browser information as part of this record.
